The ideal candidate will hold a WA RN license, active CPR, 5 years acute care nursing experience, and preferred ACM/CCM certification, with proficiency in EPIC EMR and care coordination tools.

Qualifications

• Minimum of a bachelor’s degree in nursing.
• Master’s degree in nursing preferred.
• Licensed as a Registered Nurse in the state of Washington.
• Current Healthcare Provider CPR certification required.
• ACM or CCM certification preferred.
• Minimum 5 years’ experience in acute care nursing.
• 3 years’ experience in discharge planning or case management preferred.
• Strong knowledge of discharge planning, community resources, and payer requirements.
• Proficiency in EPIC EMR and clinical decision support tools such as MCG and InterQual.

Responsibilities

• Coordinate assessment, planning, implementation, monitoring, and evaluation of care.
• Develop patient-centric treatment plans and ensure safe, cost-effective care.
• Collaborate with health care team to ensure timely post-acute care access.
