Job summary

Qualifications

• Active California RN License required.
• Minimum three years in case management or discharge planning.
• Bi-lingual in Spanish, Vietnamese, Chinese, or Tagalog is a plus.

Responsibilities

• Perform comprehensive assessments and document progress toward goals.
• Maintain case files per policies and regulatory standards.
• Develop member-centered care plans with prioritized goals.
• Coordinate with PCP, specialists, and care team to support Independence.
• Communicate with care team and providers to coordinate benefits and services.
• Facilitate transitions of care across settings and services.
• Support CBAS or MLTSS program transitions when applicable.
• Conduct home and community-based visits as needed.
• Collaborate on quality improvement and utilization management efforts.
• Participate in meetings and training as required.
