Job summary

Manatee Memorial Hospital in Bradenton, FL seeks an experienced discharge planning coordinator to lead multidisciplinary care planning across the continuum. You will organize patient care sequences to maximize outcomes and coordinate discharge with clinical teams and payers.

You will promote patient wellness and efficient utilization of services while communicating with leadership and ensuring accurate data reporting.

Qualifications

• High school diploma or GED required.
• Florida RN license required.
• Case management certification preferred.
• Discharge planning and/or utilization (InterQual) experience preferred.

Responsibilities

• Lead multidisciplinary planning and coordination of patient care across the continuum.
• Organize sequencing of patient care to maximize effectiveness, quality and cost outcomes.
• Coordinate discharge planning and home/outpatient services with the clinical team.
• Communicate with leadership and provide data reporting when required.
