Job summary

Singing River Health System Gulfport is seeking a Case Manager to coordinate patient care, focusing on clinical quality and resource management across ages within the system.

The role requires nursing licensure in Mississippi, BCLS and de-escalation training, and 2 years of relevant nursing experience in ambulatory or acute care. Strong communication, analytical, and organizational skills are essential.

Qualifications

• Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Currently licensed to practice as a Registered Nurse in Mississippi.
• Must complete and maintain BCLS certification upon hire and/or transfer; TB certification preferred; de-escalation training by orientation (90 days).
• Two (2) years’ nursing experience in ambulatory or acute care relevant to Case Management.

Responsibilities

• Coordinate patient care to improve clinical satisfaction and financial outcomes.
• Participate in daily discharge planning meetings; address barriers to discharge and transition between care levels.
• Evaluate Medicaid client needs, build care plans, and coordinate home and community-based services when responsible.
• Maintain seamless communication between inpatient care management and outpatient case management teams.
