Qualifications

• Must have RN licensure in Connecticut.
• Baccalaureate degree in clinically related field required.
• Three years of relevant clinical experience required.

Responsibilities

• Coordinate and ensure the plan of care across the continuum of care.
• Link inpatient care with outpatient services and case management.
• Optimize efficiency of hospital systems impacting quality and length of stay.
• Assist clinicians in documenting admissions and discharge decisions.
• Identify trends and barriers to care and advocate for resources.
