Job summary

Yale New Haven Health at Home is seeking a Registered Nurse Case Manager to oversee admission to home care services and develop individualized plans of care. The role requires coordinating with providers and disciplines to ensure safe delivery of care in patients' homes.

Qualifications include an RN with CT license, 2 years in acute care, CPR, and reliable transportation. The nurse will educate patients, document promptly, and participate in after-hours coverage when needed.

Qualifications

• Nursing Diploma or ADN; BSN preferred.
• Minimum two years of nursing experience in an acute care setting.
• Connecticut RN license or eligibility; CPR certification required; valid driver license and vehicle access.

Responsibilities

• Assess/reassess patients and establish patient-specific plans of care.
• Collaborate with providers and other disciplines; communicate changes promptly.
• Assess payment sources and ensure ongoing authorizations as needed.
• Educate patients and caregivers in ongoing medical maintenance; begin discharge planning on admission.
• Complete all documentation within 24 hours with changes in condition and follow-ups.
• Coordinate and manage care for patients receiving services at home; participate in case management conferences.
