• Develop individualized care plans with patients and professionals.
• Maintain accurate records of patient interactions and plans.
• Provide information about services, resources and strategies to manage conditions.
• Monitor progress and adjust discharge plans as needed.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
