Job summary

Qualifications

• Active RN in the state of IL is required.
• ADN or higher with RN licensure.
• CPR or BLS certification within 90 days of hire.

Responsibilities

• Advocate for patients’ needs and rights within healthcare and social services.
• Conduct comprehensive assessments to determine patient needs and goals.
• Coordinate services and referrals to providers and community resources.
• Deliver and explain termination of benefits letters and patient rights.
• Develop individualized care plans with patients and professionals.
• Maintain accurate records of patient interactions and plans.
• Provide families with information about services and strategies to manage conditions.
• Monitor progress and adjust discharge plans as needed.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
