Job summary

Qualifications

• Massachusetts RN licensure required.
• Three years of discharge planning or case management experience.
• Proficiency with hospital/clinical data systems and reporting.
• Excellent organizational and communication skills.

Responsibilities

• Assess patients' physical, functional, and psycho-social needs using screening tools and collaboration with care teams.
• Perform admission and concurrent reviews per payer requirements; coordinate with third party payers as needed.
• Collaboratively facilitate care across the continuum; support Care Coordination Team and identify improvement opportunities.
• Develop individual care plans with patient/family to meet goals and level of care.
• Contact third party payers to clarify benefits, coverage, and resources; negotiate where needed.
• Coordinate, monitor and evaluate discharge plans and communicate progress to involved parties.
• Educate patients, families and teams about resources and decision-making options.
• Contribute to quality improvement initiatives and compliance with regulations and standards.
• Maintain professional standards and participate in department-wide performance improvement activities.
