Qualifications

• Bachelor’s Degree in nursing required, must start within 1 year and complete within 5 years.
• Broad experience in clinical, administrative and managerial roles in healthcare or related field.
• RN license valid in Indiana.

Responsibilities

• Discharge screening, evaluation, development and implementation of discharge plans with focus on those with complex needs.
• Reassessment of discharge plan and monitoring for changes in condition
• Patient advocacy with support persons and healthcare team as needed
• Interventions and support with family stress and emotional needs related to healthcare decisions
• Crisis intervention/adjustment to illness/bereavement
• Resource brokering (SNF, home care, hospice, infusion, LTAC, DME, etc)
• System integration/continuity of care and interdisciplinary collaboration
• Follow up calls per Transitions of Care Discharge Policy and telephone assessments
• Provision of resources before/after admission to facilitate discharge planning
• Medicare notification letters and financial assistance navigation
• Abuse assessments and referrals; Social Determinants of Health assessment
• Advance directives information and execution; participation in department/community meetings

Education

Bachelor’s Degree in nursing
