Qualifications

• Bachelor's of Nursing RN, PA-licensed.
• Licensed Registered Nurse in Pennsylvania.
• Minimum 3 years RN experience in Case Management or Discharge Planning.

Responsibilities

• Coordinate discharge planning for defined patients.
• Advocate for the patient and family throughout the episode of care.
• Precept and mentor new staff members.
• Document all relevant information in the Medical Record.
• Facilitate referrals and authorizations for post-acute care.
