Job summary

UMass Memorial Health is seeking a skilled discharge planning and case management RN to identify patient needs, develop care plans, and coordinate resources across the continuum of care. The role emphasizes data-driven care coordination and collaboration with physicians and care teams.

Responsibilities include inpatient reviews, educating providers on care levels, and ensuring efficient discharge processes while supporting quality improvement and regulatory compliance.

Qualifications

• Massachusetts licensure as a registered nurse.
• Three (3) years clinical experience in discharge planning or case management.
• Ability to use specialized computer-based systems for gathering, reporting, and analysis of clinical data.
• Excellent organizational and communications skills.

Responsibilities

• Assess patient’s physical, functional, and psycho-social needs and develop plans of care.
• Perform admissions and concurrent reviews per payer requirements.
• Collaborate with Care Coordination Team to monitor level of care and plan improvements.
• Coordinate discharge planning and ensure timely communication with payers and resources.
• Support quality improvement and regulatory compliance.
