The Case Manager promotes quality of care, supports resource utilization, and plans for safe discharge to the most appropriate level of care. The role requires collaboration with a multidisciplinary team, adherence to HIPAA, and ongoing assessment to meet clinical and financial outcomes.

Qualifications

• Bachelor’s degree in Nursing required.
• PA RN license required.
• Complete required competencies and trainings as per role.

Responsibilities

• Maintaining safe, high-quality patient care and ensuring timely coordination of services.
• Oversee caseload management, follow-up, and address patients' complex needs.
• Manage and maintain appropriate caseloads, support continuity of care and timely follow-up.
• Collaborate with the multi-disciplinary care team to achieve clinical, financial and quality outcomes.
