Job summary

Wythe County Community Hospital is seeking an Registered Nurse (RN) – Case Manager to join our team on a full-time basis with day shifts and on-call requirements. The RN will collaborate with the multidisciplinary team to assess patient needs, coordinate admission, discharge planning, and transfers, and ensure education and resources are provided for successful transitions.

Ideal candidates will hold an active Virginia RN license and BLS certification, with 3+ years of RN clinical experience and

Qualifications

• Active Virginia RN license (single-state) or compact license.
• Basic Life Support certification within 30 days of hire.
• 3 years RN clinical experience preferred.
• RN/Case Management experience preferred.

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and appropriateness of care.
• Monitors patient and family satisfaction; responds to questions and complaints.
• Leads and coordinates discharge planning including patient transfers and referrals to community resources.
• Performs chart reviews.
• Documents case management activities, plans and outcomes.
• Supports department operations by working alternate shifts/units; may be on-call.
