Job summary

Wythe County Community Hospital, part of Lifepoint Health, is seeking a Registered Nurse (RN) Case Manager for a full-time day schedule with on‑call requirements. You will collaborate with the multidisciplinary team to coordinate admissions, discharges, and patient transitions to community resources.

The role emphasizes care coordination, chart reviews, and ensuring high-quality outcomes while supporting department operations.

Qualifications

• Active Virginia RN license (single‑state) or compact license.
• BLS certification required within 30 days of hire.
• 3 years of RN clinical experience preferred.
• Previous RN/Case Management experience preferred.

Responsibilities

• Consults regularly with nursing staff and multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints.
• Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education.
• Performs chart reviews.
• Accurately documents case management activities, plans and outcomes.
• Supports department operations by working alternate shifts/units as needed; may be assigned on-call.

What we're looking for

Applicants should have an active Virginia RN license (single‑state) or compact license. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins.

Additional requirements include:

• Basic Life Support certification is required within 30 days of hire
• Previous RN/Case Management experience is preferred
• 3 years of RN clinical experience preferred
