Job summary

Wythe County Community Hospital is seeking a Registered Nurse (RN) Case Manager in Wytheville, VA. This full-time, days-shift role includes on-call requirements and entails coordinating patient care, discharge planning, and documentation of outcomes.

A Virginia RN license and BLS are required, with prior RN/case management experience preferred. The position emphasizes collaboration with nursing and multidisciplinary teams to evaluate admission status, ensure timely transfers, and support

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and appropriateness of care (admission, length of stay, transfers, discharge).
• Monitors patient and family satisfaction; responds to questions and complaints.
• Leads and coordinates discharge planning, including transfers and referrals to community resources; provides discharge education.
• Performs chart reviews.
• Accurately documents case management activities, plans and outcomes.
• Supports department operations by working alternate shifts/units as needed; may be assigned on-call.

How you'll contribute

What we're looking for

Applicants should have an active Virginia RN license (single-state) or compact license. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins.

Additional requirements include:

• Basic Life Support certification is required within 30 days of hire
• Previous RN/Case Management experience is preferred
• 3 years of RN clinical experience preferred
