Job summary

VOLTO Consulting is seeking an RN Case Manager to support hospital-based discharge planning in Morristown, NJ. The role focuses on coordinating patients through the acute care hospitalization and ensuring smooth transitions to next care settings.

The position requires a BSN and RN license, with CCMC preferred, and involves 2 years of acute care discharge planning experience, rotating weekends, and holiday coverage.

Qualifications

• BSN is required.
• RN license required.
• Experience with discharge planning and placement in rehabilitation facilities.
• Minimum 2 years of acute inpatient hospital discharge planning.
• Rotating weekends: every 4th or 5th weekend.
• Holidays: one summer and one winter holiday.
• Experience with subacute patients.
