Qualifications

• Registered Nurse with TX state license.
• Associate Degree in Nursing or Nursing Diploma required; Bachelor's preferred.
• 3+ years of clinical hospital nursing or 2+ years in case management.
• Case management certification preferred.

Responsibilities

• Conduct comprehensive psychosocial, medical, and discharge assessments.
• Reassess patient condition and plan readmission risk mitigation and education.
• Coordinate care plan and discharge planning with the multidisciplinary team.
• Partner with Social Services to address post-acute needs and resources.
• Refer to third party payer, disease management, and case management programs.
• Facilitate patient throughput and effective care transitions.
