Job summary

Houston Methodist is seeking a Part-Time Case Manager (RN) to plan comprehensive case management, discharge planning, and transitions for a targeted patient population. You will work with physicians and an interprofessional team to ensure medically necessary admissions and appropriate post-acute dispositions.

Required: RN license in Texas with Texas PSAV Compact/Licensure within 60 days if relocating. Three years of hospital nursing and case management experience preferred.

Qualifications

• Three years hospital nursing clinical experience.
• Case management experience preferred.
• RN licensure in Texas required; permanent license within 60 days if establishing Texas residency.

Responsibilities

• Communicates care data to health care team and families clearly and comprehensively.
• Facilitates discharge planning with social worker and care team for complex discharges.
• Initiates referrals for home health, hospice, and durable medical equipment; coordinates transitions.
