Qualifications

• 3 years Acute Care experience required.
• Active Licensed Registered Nurse (RN).
• Basic Life Support (BLS) certification required.
• All post high school education must have been obtained from a recognized College or University.
• Associates Nursing required; Bachelors Nursing preferred.

Responsibilities

• Applies to chart review to determine medical necessity for admission, continued stay, and discharge.
• Collects data for clinical improvement initiatives and action planning.
• Conducts concurrent chart reviews for complete documentation.
• Coordinates plan of care with health care team, patient, caregiver, and family.
• Performs discharge planning to appropriate level of care and services.
• Communicates effectively with payers, physicians, and care team.
• Performs other duties as needed.
