Qualifications

• Graduate of an accredited nursing program.
• Current Texas RN licensure (or NLC) and BLS certification per AHA/ARC.
• Bachelor's Degree in Nursing preferred; two years RN experience and acute care setting ideal.

Responsibilities

• Leads multidisciplinary team to develop, refine, and communicate care plans for defined patients.
• Implement clinical pathways and monitor education and discharge needs.
• Coordinate with social work, utilization review, and other teams to optimize resources.
