Qualifications

• RN with a valid, unrestricted State of Michigan license.
• Minimum 3 years of clinical experience.
• Certification in Case Management (CCM) or ACM within 3 years of hire.
• Experience in discharge planning, home health care, or managed care.
• Knowledge of Medicare and Medicaid regulations and case management principles.
• Excellent customer service and interpersonal skills.

Responsibilities

• Assess patients and family needs to guide recovery planning.
• Develop and monitor a patient-centered care plan with an interdisciplinary team.
• Coordinate care transitions and ensure safety and quality.
• Provide counseling on treatment decisions and end-of-life care planning.
• Collaborate with community resources to meet complex social needs.
