Job summary

PACS Group is seeking a dedicated Nursing/Case Management professional to collaborate with residents, families, and care teams to develop and implement cost-conscious, high-quality plans of care. The role emphasizes timely documentation and interprofessional coordination within a hospital or facility setting.

Ideal candidates will hold a Nursing or Social Work degree with RN license preferred, and have experience with Medicare/Medi-Cal/Medicaid programs.

Qualifications

  • Bachelor's Degree in Nursing or Social Work; RN license preferred.
  • Two years of clinical nursing experience preferred.
  • Knowledge of Medicare, Medi-Cal and Medicaid programs.
  • PCC Knowledge preferred.
  • CCM certification a plus.
  • Continuing education/licensing required.

Responsibilities

  • Communicate with residents and families about Plans of Care and treatment protocols.
  • Attend morning meetings to facilitate team communications.
  • Answer residents' questions about care, illness progression, and plans.
  • Monitor and adjust resident statuses based on changing needs.
  • Complete documentation and charting promptly.
  • Coordinate care with providers, facilities, and payers.
  • Oversee admissions and discharge activities.
