Qualifications

  • Bachelor's Degree in Nursing or Social Work required.
  • RN license preferred; LVN/LPN acceptable.
  • Two years of clinical nursing experience preferred.
  • Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
  • Ability to read policies and procedures and communicate with managers and employees.
  • Must maintain continuing education/licensing.

Responsibilities

  • Communicate with residents and families about plans of care and treatment protocols.
  • Attend morning meetings to facilitate team communications.
  • Monitor residents' statuses and adjust care as needs change.
  • Organize and prioritize daily work for residents in responsibility areas.
  • Complete documentation and timely paperwork.
  • Coordinate admissions, discharges and referrals with care providers and payers.
  • Perform utilization review to ensure appropriate, cost-effective care.
  • Work independently with sound judgment in interactions with physicians and staff.
