Duly Health and Care in Illinois is seeking an RN Case Manager to collaborate with physicians, patients, and families to promote quality outcomes for patients with complex medical needs. You will coordinate care across settings to optimize utilization and prevent inpatient readmissions.

The role requires an active RN license in Illinois, 2–4 years of related experience, and CPR/BLS certification; a BSN is preferred.

Qualifications

• RN with Illinois license is required.
• Experience in primary care or hospital setting preferred.
• Case management experience preferred.

Responsibilities

• Collaborate with physicians, patients, and families to promote quality outcomes.
• Identify high-risk cases and coordinate intensive case management.
• Manage patient care across settings to prevent unnecessary hospitalizations.
• Document case management activities and communicate with health plans.
• Coordinate services and resources to meet medical and psychosocial needs.
