Job summary

Duly Health and Care is seeking an RN Case Manager for the Care Ally program in Downers Grove, IL. You will work with physicians, patients, and families to optimize outcomes for patients with complex needs.

The role requires an active IL RN license, BLS/CPR, and 2–4 years in primary care or hospital settings. Expect a full-time schedule, with potential for flexible hours and ongoing education support.

Qualifications

• Must hold an active Registered Nurse license in Illinois.
• Certification in Case Management is preferred.
• Current CPR/BLS certification required.

Responsibilities

• Collaborate with physicians, patients and families to promote quality outcomes.
• Manage a caseload of patients with complex medical needs.
• Develop individualized case management plans with prioritized goals.
• Coordinate health care services with physicians and providers.
• Document communications, attestations, and case closures per policies.
