Evara Health in Pinellas Park, FL is seeking a dedicated Nurse Case Manager to coordinate patient care, educate patients on behavioral health and chronic conditions, and support population health initiatives. The role emphasizes documentation, compliance, and collaboration with a multidisciplinary team.

The ideal candidate will be a graduate of an accredited RN program with an active Florida RN license and at least two years of RN practice, including pediatric exposure.

Qualifications

• Graduate of an accredited RN program and an active Florida RN license.
• Minimum two years as an RN in a healthcare setting; supervisory experience preferred.
• Two years with pediatric population preferred; Certified Case Manager preferred.

Responsibilities

• Coordinate care for patients and families by connecting them with providers, specialists, behavioral health services, and community resources.
• Develop, monitor, and update individualized care plans to support patient-centered goals and improve health outcomes.
• Provide individual and group education on behavioral health, chronic conditions, healthy behaviors, and available Evara Health services.
• Engage patients and families in their care, helping them navigate services and overcome barriers to treatment.
• Identify high-risk patients, monitor progress, and collaborate with care teams to ensure appropriate care levels.
• Assess service utilization, identify gaps in care, and support initiatives that improve quality and outcomes.
• Partner with providers, therapists, nurses, and care team members to coordinate comprehensive patient care; participate in huddles and workflow improvements; serve as a resource for staff and partners.
• Maintain accurate documentation and ensure timely follow-up in line with policies; deliver exceptional customer service while adhering to HIPAA and regulations.
