Evara Health is seeking a Case Manager- Behavioral Health to provide assessment, care coordination, and ongoing support for patients with behavioral health needs in Clearwater, FL. You will work with providers to promote engagement in therapy, treatment adherence, and patient satisfaction.

Qualifications

• Bachelor’s degree from an accredited four-year college or university in social work, psychology, or a human services field.
• Certified Case Manager preferred but not required

Responsibilities

• Assess social determinants of health to develop patient-centered care plans.
• Coordinate internal and external referrals for behavioral health services and related resources.
• Conduct in-person and virtual check-ins to monitor progress and treatment goals.
• Maintain accurate EHR documentation and program outcomes.
• Collaborate with patients, families, and providers while ensuring HIPAA compliance.
