Job summary

LanceSoft is seeking a Case Manager RN for a day shift in Wakulla Springs, FL. The RN CM Care Coordinator will identify barriers to patient throughput and quality outcomes and will facilitate appropriate discharge plans.

Ideal candidates have 3 years of recent acute care Case Manager experience or 3 years in Med/Surg, Tele, Neuro, ICU, PCU, or ED, with an Associates or Diploma in Nursing and an active FL RN license (or compact).

Qualifications
