LanceSoft is seeking an experienced RN Case Manager to join a leading acute care hospital in the Florida Panhandle. The role focuses on patient advocacy, discharge planning, and coordinating high-quality care across the patient journey.

Ideal candidates are organized and comfortable communicating with physicians and families in a collaborative hospital environment. Full-time daytime schedule with rotating weekends; generous compensation and a $10,000 sign-on bonus.

Qualifications

• Active RN license
• Associate Degree or Nursing Diploma
• Minimum 2 years of recent acute care experience within the last 3 years
• Background in ICU, CCU, Med-Surg, LTAC, or Hospital Case Management preferred
• Strong communication, critical thinking, and organizational skills

Responsibilities

• Coordinate patient care plans across the healthcare continuum
• Manage discharge planning and psychosocial assessments
• Educate patients and families regarding treatment goals and post-discharge care
• Collaborate with physicians, nurses, and interdisciplinary teams
• Advocate for safe and appropriate patient outcomes
• Ensure timely documentation and care coordination
