Job summary

LanceSoft is seeking an experienced Registered Nurse Case Manager to join a collaborative healthcare team in Florida. You will coordinate patient care, facilitate discharge planning, and connect patients with resources, working with interdisciplinary teams to ensure high-quality outcomes throughout the continuum of care.

Schedule: Day shift 8:00 AM – 4:30 PM with rotating weekends. Compensation: $34.25–$47.10/hour, sign-on bonus and relocation assistance on a case-by-case basis.

Qualifications

• Active RN license is required.
• Associate Degree in Nursing or Nursing Certificate required.
• Minimum 2 years of recent Acute Care experience within the last 3 years.
• Experience in ICU/CCU/Med-Surg/LTAC/Hospital Case Management preferred.
• Strong critical thinking, communication, and organizational skills.
• Ability to collaborate with physicians and multidisciplinary teams.
• Comfortable advocating for patients in complex discharge planning discussions.

Responsibilities

• Guide patients and families through orientation, discharge planning and resources.
• Explain Medicare, insurance benefits, patient rights, and discharge criteria.
• Coordinate education for patients, families and caregivers to support treatment planning.
• Develop and implement individualized care and discharge plans.
• Complete psychosocial assessments and discharge planning evaluations.
• Collaborate with physicians, nursing staff and interdisciplinary teams for safe transitions.

Qualifications

• Experience in one or more of the following preferred:
