Qualifications

• Active RN license or APRN license required.
• Associate Degree in Nursing or RN Diploma required.
• Minimum 2 years of recent acute care RN Case Management experience (within the last year) required.
• Strong care coordination, discharge planning, and patient advocacy skills required.

Join a patient-focused healthcare team as a Registered Nurse Case Manager and play a critical role in coordinating care, improving patient outcomes, and ensuring seamless transitions throughout the healthcare continuum. This position offers the opportunity to work closely with physicians, nurses, patients, and families to develop individualized care plans, facilitate discharge planning, and connect patients with the resources they need for successful recovery.

Key Responsibilities

Compensation & Benefits

• Pay Range: $36.96 – $44.00 per hour (based on experience)
• Weekend Differential: $1.50 per hour
• Sign-On Bonus: Available on a case-by-case basis
• Relocation Assistance: Available on a case-by-case basis

Schedule: Full-Time - Day Shift
Shift Options:

• 8:00 AM – 4:30 PM
• 9:00 AM – 5:30 PM
• 10:00 AM – 6:30 PM
• 11:00 AM – 7:30 PM

Monday – Friday *Rotating Weekends (typically every other Saturday, potentially every third Saturday when fully staffed)
