University Health seeks a Nurse Case Manager to coordinate the treatment plan for patients during acute care episodes in collaboration with the patient and the interdisciplinary team.

You will proactively facilitate interventions to ensure timely delivery of services, evaluate effectiveness, and track barriers in the plan of care.

As a patient advocate, you will identify health care needs and communicate them clearly to the care team.

Qualifikationen

• Bachelor's degree in Nursing is highly preferred.
• Three to five years nursing experience required (Staff nurse II or above).
• Experience in case management, utilization review or hospital quality is preferred.
