Job summary

University Health is seeking a nurse case manager to coordinate treatment plans within the acute care setting. The role involves collaborating with patients and an interdisciplinary team to ensure timely delivery of services and to advocate for patient needs.

The ideal candidate will have a Texas RN license and 3–5 years of nursing experience, with preferred experience in case management, utilization review, or hospital quality.

Qualifications

• Bachelor's degree in Nursing required or highly preferred.
• RN licensure in Texas required.
• 3–5 years nursing experience; experience in case management, utilization review or hospital quality preferred.
• ACM/CCM/ANCC certification preferred and must be achieved within 2 years of placement.
• ACLS/BLS or health care provider card preferred.

Responsibilities

• Coordinate treatment/plans for patients within acute episodes of care in collaboration with patient and interdisciplinary team.
• Proactively facilitate interventions to ensure timely delivery of services.
• Evaluate effectiveness of interventions and track variances or barriers in the plan of care.
• Advocate for patients and communicate health care needs to the team.

Education

Bachelor's degree in Nursing

RN licensure Texas

Case management experience (preferred)

Case management certification (ACM/CCM/ANCC) preferred

BLS/Health Care Provider card preferred
