Job summary

Elara Caring is seeking a Registered Nurse Home Health to join our Portland, ME team as RN Case Manager. You will assess patients, develop care plans, coordinate with the care team, and educate families to support safe, high-quality home care.

A $5,000 sign-on bonus is available and ongoing professional growth is encouraged. Requirements include an active RN license, a nursing degree, at least 1 year of clinical experience, and the ability to travel to patient homes.
