Job summary

Elara Caring is seeking a full-time RN Case Manager to serve patients in Greenwich and Stamford, CT. You will assess, coordinate, and deliver home health care, partnering with a clinical team to optimize outcomes.

Requirements include an active CT RN license and at least 1 year of clinical nursing experience; home health is preferred. A valid driver's license or reliable transport is needed for travel between patient homes.

Qualifications

• Current Connecticut license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred, but not required.

Responsibilities

• Complete initial and ongoing assessments to identify patient care needs.
• Provide skilled nursing care according to individualized care plans and physician orders.
• Monitor patient conditions, communicate changes, and take appropriate action.
• Coordinate with physicians and the care team to support safe, continuous care.
• Educate patients and families about their care, health conditions, and safety.
• Help patients manage their health and reduce avoidable hospital visits.
• Protect patient and staff privacy in accordance with HIPAA.
