Job summary

Heart to Heart Hospice is seeking an RN Case Manager to lead comprehensive patient assessments and coordinate care plans for hospice patients. You will collaborate with physicians, caregivers, and the IDG to ensure timely, compassionate, and compliant care delivery.

The role requires an RN license and nursing degree or diploma, with preferred two years of RN experience in a clinical or healthcare setting. A valid driver's license and reliable transportation are required.

Responsibilities

• Initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs and ADLs.
• Implement and adjust the individualized plan of care.
• Coordinate care among patient, family, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning.
• Provide on-call service to patients and families as assigned.
• Timely submission of all documentation.
• Ensure compliance with applicable laws and policies.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
