Job summary

Heart to Heart Hospice is seeking an RN Case Manager to conduct comprehensive assessments and develop individualized care plans for terminally ill patients in the Humble, TX area.

You will collaborate with an interdisciplinary team, coordinate care, and participate in IDG meetings while providing on-call support as needed. Strong nursing credentials and reliable transportation are required.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
• Current State Registered Nurse License (RN).
• Valid driver's license and reliable transportation.

Responsibilities

• Initial and ongoing comprehensive assessments of patients with terminal illness, addressing physical, psychosocial, environmental needs and ADLs.
• Implement and revise the individualized plan of care (POC) as needed.
• Collaborate with patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with the team.
• Provide on-call coverage to patients and families as assigned.
• Timely submission of all documentation and compliance with laws and policies.
• Perform other duties as required by department and role.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
