Job summary

Heart to Heart Hospice is seeking an RN Case Manager to lead comprehensive assessments of patients facing terminal illness and coordinate their care plans. You will work closely with physicians, patients, families, and the IDG to ensure timely, compliant, and compassionate care.

The role emphasizes on-call availability, documentation, and adherence to Medicare regulations, with benefits including competitive pay, medical/dental/vision insurance, PTO, and a company car for eligible staff.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized Plan of Care (POC) and revise as necessary.
• Collaborates care among patient/patient caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and coordinate patient care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Ensure compliance with local, state and federal laws, Medicare regulations, and established policies.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
