Job summary

Elara Caring is seeking a Home Health Registered Nurse to join our full-time team serving West Salem, Wooster, and Wadsworth. You will provide skilled nursing care, build meaningful patient relationships, educate families, and partner with a clinical team to support recovery and independence at home.

You’ll use your nursing skills to assess patient needs, create and follow care plans, and monitor conditions while upholding HIPAA, safety, and quality standards.

Qualifications

• Current state license as a Registered Nurse.
• Graduate of an accredited nursing program.
• At least 1 year of nursing experience in a clinical setting.
• Home health experience preferred, but not required.
• Ability to frequently lift, push, pull, and assist patients up to 50 pounds.
• Valid driver’s license and reliable transportation to travel between patient homes.

Responsibilities

• Perform initial and ongoing patient assessments to determine care needs.
• Provide skilled nursing care per individualized care plans and physician orders.
• Monitor patient conditions, report changes, and take appropriate action.
• Educate patients and families about care, conditions, and safety.
• Coordinate with the care team to ensure safe, continuous patient care.
• Protect patient and staff privacy in accordance with HIPAA.

What You’ll Need

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
