Job summary

Elara Caring is seeking a Registered Nurse Home Health to join our elite team delivering Right Care, at the Right Time, in the Right Place. You will assess patients, complete initial and ongoing assessments, and educate families on safety and care.

The role requires an active RN license, a nursing degree, 1+ year of clinical nursing experience, and the ability to lift up to 50 pounds. A driver’s license or reliable transport is needed for travel between homes.

Qualifications

• Current state RN license.
• Graduate of an accredited nursing program (certificate/diploma/ASN/BSN).
• 1+ year of clinical nursing experience.
• Home health experience preferred.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Assess patients to determine care needs.

As a Registered Nurse Home Health, you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.


• Completing initial and ongoing patient assessments.


• Protecting patient and staff privacy in accordance with HIPAA.


• Providing nursing care according to the plan of care and physician orders.


• Monitoring patient conditions and reporting changes to the care team.


• Educating patients and families on their care and safety.


• Taking appropriate action when patient conditions change.


• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.


• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.


• 1+ year of experience as a nurse in a clinical care setting.


• Home Health experience is preferred, but not required.


• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment


• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
