Job summary

Tennessee Quality Care Hospice in Waynesboro, TN is seeking an RN Case Manager to join our growing team. This field-based role covers Waynesboro and surrounding counties, focusing on therapeutic nursing and coordinating home-based care for patients and families.

You will conduct assessments, develop and implement plans of care, collaborate with the interdisciplinary team, and provide regular visits. On-call duties 4–8 times monthly and occasional weekend hours are required.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload across nursing process phases.
• Obtain data on patient factors and initiate intervention with the interdisciplinary team.
• Coordinate communication with attending physician and agencies for optimal care.
• Provide regular visits to reassess needs and update the plan of care.
• Document care plans and supervise Home Health Aide delivery per guidelines.

Qualifications

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid drivers license and proof of insurance is required

What You’ll Do

• Assume primary responsibility for patient and family caseload including the assessment determining eligibility admission processing planning implementation and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact the patient and familys needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re assess the patients needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records charts progress notes reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal State and Company guidelines
