Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join our growing team serving Martin, TN and surrounding counties. This field-based role manages a patient/family caseload, coordinating care and supporting families in the home setting.

Responsibilities include nursing process administration, data gathering on medical and psychosocial factors, and collaboration with the interdisciplinary team. On-call rotations apply with regular home visits and documentation.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid drivers license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload including the assessment, determining eligibility, admission, processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact the patient and family’s needs and initiate intervention by other members of the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team and other agencies for coordinating optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State and Company guidelines.

Territory Martin TN and surrounding counties. Schedule Monday Friday 8am 430pm. On Call 4 5 times per month.
