Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their team in Jackson, TN. This field-based role manages a patient load, coordinates care, and supports patients and families in home settings.

Responsibilities include comprehensive nursing care, care planning, and collaboration with physicians and the interdisciplinary team to ensure optimal home-based hospice services.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance is required.

Responsibilities

• Manage a patient and family caseload through assessment, eligibility, admission, planning, implementation and evaluation of the nursing process.
• Obtain data from patient physical, psychological, social, and spiritual factors and initiate interdisciplinary interventions.
• Coordinate care by communicating with the attending physician and other agencies.
• Provide regular home visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and supervise Home Health Aide delivery according to guidelines.

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Jackson.

This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory:

Jackson, TN and surrounding counties

Schedule:

Monday thru Friday 8am-4:30pm.

What You'll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
