Job summary

Tennessee Quality Care Hospice seeks an RN Case Manager to join our growing team in Martin, TN. This field-based role manages a home health caseload, coordinating care and supporting patients and families in their homes.

Territory includes Martin and surrounding counties; schedule is Mon-Fri 8am-4:30pm, with on-call 4–5 times per month. The position offers comprehensive benefits including medical/dental/vision coverage from the first of the month, a 401(k) with company match, generous PTO and

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Manage a patient and family caseload through assessment, admission, planning, implementation and evaluation phases of the nursing process.
• Gather data from patient, family and interdisciplinary team to determine needs.
• Coordinate care by communicating with attending physician and other agencies.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and support family.
• Maintain accurate patient records and care plans.

Job description

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Martin. This field based position is responsible for managing a caseload of patients providing therapeutic nursing coordinating care and supporting patients and their families in the comfort of their homes. Territory Martin TN and surrounding counties. Schedule Monday Friday 8am 430pm. On Call 4 5 times per month.

Qualifications

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid drivers license and proof of insurance is required

What Youll Do

• Assume primary responsibility for patient and family caseload including the assessment determining eligibility admission processing planning implementation and evaluation phases of the nursing process.
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact the patient and familys needs and initiate intervention by other members of the interdisciplinary team.
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care.
• Provide regular visits to re assess the patients needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records charts progress notes reports and prepare care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by Federal State and Company guidelines.
