POSITION SUMMARY: RN Case Manager

POSITION DUTIES : Discharge Planning

MINIMUM REQUIRED QUALIFICATIONS: RN; 3+ years minimum of acute care hospital experience

PREFERRED QUALIFICATIONS: RN, BSN

LENGTH OF ASSIGNMENT: 13 weeks
