Cynet HealthStaff is seeking an RN - Case Manager for a 13-week assignment on a day shift with 8-hour shifts. You will oversee patient care, discharge planning, and needs assessments for adults and geriatrics, ensuring adherence to care plans and guidelines.

Description:Targeted start date is 10/5/2026, will also consider 9/21/2026 starts.The RN - Case Manager is responsible for overseeing the care of patients.The ideal candidate must have experience in case management and discharge planning.Patient age groups include adults and geriatrics.Participation in weekend shifts is required, with one weekend day per month.Applicant must comply with all HEALTHCARE guidelines and standards.Experience with charting systems is preferred.Understanding of CMS and HIPAA guidelines is necessary.Candidate must be capable of conducting needs assessments and order durable medical equipment.They must demonstrate skills in creating and managing plans of care.Interdisciplinary support is available for patient care.The patient ratio for day shifts is estimated at 15 patients per nurse.Attendance policy allows for up to three absences per 13-week contract.Excessive tardiness or any no-call/no-show may lead to further action.Modules are completed during the orientation and annually thereafter.Compliance deadlines and attendance will be closely monitored.A supervisory reference within the last year is required for all applicants.Candidates must have an active nursing license and relevant certifications in hand when applying.
