Qualifications

• Discharge planning experience in an acute care hospital preferred (large org/teaching).

POSITION SUMMARY – RN Case Manager

Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

POSITION DUTIES

• MA RN license
• AHA BLS
• 2+ years of case management experience
• 6-months discharge planning Case Management experience in an Acute Care hospital (ideally be a large organization/teaching hospital with 500 beds or more)

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

M-F, Days. Dependent on operational need, may be 4x10's or 5x8's.
