Job summary

Bristol Hospice is seeking a dedicated Registered Nurse Case Manager to provide compassionate, whole-person care for patients and their families in Tucson, AZ. This full-time, hourly position emphasizes advocacy, comfort, and dignity on the patient’s hospice journey.

You will develop individualized care plans, monitor vitals, administer medications, educate families, and collaborate with interdisciplinary teams to ensure comprehensive care.

Qualifications

• A valid Registered Nurse license in good standing.
• A valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.
• Flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned
