Job summary

Bristol Hospice Inland Empire in Riverside, CA is seeking a Registered Nurse Case Manager who provides compassionate, whole person care and guides patients and families through the hospice journey.

You will develop individualized care plans, monitor vital signs, administer treatments, educate families, and collaborate with an interdisciplinary team in a mission driven culture with day shifts and hourly pay.

Qualifications

• Active RN license in good standing in the state of practice.
• Current CPR certification required.
• Valid driver's license and reliable transportation.
• Excellent written and verbal communication with families and medical teams.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Provide oneonone support to patients and families, ensuring comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs regularly and accurately.
• Administer physicianordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Track changes in patient condition and respond promptly.
• Maintain up to date clinical notes reflecting patient progress.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate, mission-driven team.
• Additional duties as assigned.
